Substance Abuse Evaluations/Mental Health Evaluations

The Substance Abuse Evaluation process with an experienced professional from Cactus includes:

1) The four page Substance Abuse Evaluation or Mental Health Evaluations is converted to readable

     paragraphs for each section.  There is a written summary at the end which                           

     creates an easier understanding of the document. Recommendations for possible 

     treatment are included.  

Once all Releases of Information are signed, the Substance Abuse Evaluation or Mental Health Evaluations process will be completed and delivered to the client within 72 hours.  This is from time of appointment to email to relevant party.  This is a turnaround that no one can match on a regular basis. The Substance Abuse Evaluation is also available for other counties in Florida with specific fees relating to distance and time.

All Substance Abuse evaluations are paid for in advance at the time the appointment is made.  The cost is $175.00 USD.  Mental Health Evaluations are $225.00 USD.  Credit cards, PayPal, and cash are accepted. Credit cards have a $5.00 USD Processing Fee.

Cancelling or rescheduling appointments for Substance Abuse Evaluations or Mental Health Evaluations, must be done 48 hours prior to the appointment or the fee is nonrefundable.  An appointment that is missed has no refundable fee.
