What is included in the Substance Abuse Evaluation?
The Substance Abuse Evaluation process with an experienced professional from Cactus includes:
1) The four page Substance Abuse Evaluation which is converted to readable
paragraphs for each section. There is a written summary at the end which
creates an easier understanding of the document. Recommendations for possible
treatment are included.
How long does the Substance Abuse Evaluation take?
Once all Releases of Information are signed the Evaluation process will be completed and delivered to the client within 72 hours. This is from time of appointment to email to relevant party. This is a turnaround that no one can match on a regular basis.
Where does the Substance Abuse Evaluation happen?
Once all Releases of information are signed, the evaluation can take place at our office at 611 Druid Road, Ste. 704 Clearwater, FL 33756 or we can also do in custody evaluations. In custody requires either attorney or family to get signed release of information.
The Substance Abuse Evaluation is also available for other counties in Florida with specific fees relating to distance and time.
How much does the Substance Abuse Evaluation cost?
All Substance Abuse evaluations are paid for in advance at the time the appointment is made. The cost is $150.00 USD. Mental Health Evaluations are $200.00 USD. Credit cards, PayPal, and cash are accepted. Credit cards have a $5.00 USD Processing Fee.
Cancelling or Missing Appointments
Cancelling or rescheduling appointments must be done 48 hours prior to the appointment or the fee is nonrefundable. An appointment that is missed has no refundable fee.
